In the personal details block, copy your title, family name, first name, age, date and place of birth, and nationality exactly as printed on your passport's bio page — mismatches here are a common cause of delay.
For passport details, enter the passport number, plus its date and place of issue and validity date exactly as shown on the bio page; note that the printed form asks for day, month, and year on separate lines, so double-check you haven't transposed them.
Next, describe your current entry: the visa type you used (Visa Exemption, Tourist Visa, Non-Immigrant Visa, etc.), how you traveled and from where, your port and date of arrival, and your Arrival/Departure Card (TM.6) number. If you arrived after 1 May 2025, there is no paper TM.6 — enter your Thailand Digital Arrival Card (TDAC) reference in that field instead.
